- • The Med-Care Direct Access Plan is a direct primary care membership, not insurance
- • Covers primary care services provided by Med-Care Providers
- • Does not include emergency room visits, specialist care, hospitalizations, or imaging
Payment Terms
- • Monthly payments processed automatically via credit/debit card or ACH
- • Payments are non-refundable once charged
- • $25 one-time enrollment fee for individual plans
Visits and Limits
- • Unlimited telehealth visits included
- • One (1) in-office visit included per month
- • Additional office visits: $49 member rate
- • No-shows or same-day cancellations may incur a $25 fee
